[KwartzLab] Creating a new page in the KWartzlab Wiki

I've searched all over and can't see how to do this. I found an "Add page with form" n "Special Pages" but when I try to use it I get the error:
Fatal error: Call to undefined function wfElement() in /home/kwartzla/public_html/mediawiki/extensions/SemanticForms/includes/SF_FormUtils.inc on line 519

(I want to add a page concerning the calibration of the 3D printer and related parameters)
